Forms to apply for college credit
The same forms are used for either university. The student must fill out the application form and send it with a check for the appropriate amount (see the description for rates) to the address indicated. The student should fill out the top portion of the Verification Form and provide it to the instructor who will fill in the scores and send it to PLTW Iowa.
